Mother in law care

My 75 Y.O. mother-in-law has Alzheimer's and is under the care of her husband and his sister.  He is now in the hospital with cancer in another town.  His sister is with him.  My wife took her to a much needed doctors appointment today but was denied care.  They said she can only be seen if the caregiver is there to sign.  The caregiver said she will attend to it when she has time away from her brother in the other town.  We do not know when this will be so my mother-in-law can not be seen unless the husband or sister has time to personally come to sign.  My wife is with her mother but can offer no help.  There must be another solution than risk someone's health due to the court assigned caregiver is too busy to attend to what they signed on for.
